About Dr. Kozak
Dr. Kimberly A. Kozak is Board-Certified through AOCOOHNS in Otorhinolaryngology and Facial Plastic Surgery.
She has been practicing in Oregon since 1997 and established her state-of-the-art private practice in Grants Pass in January 2009.
Dr. Kozak received her D.O. at Michigan State University College of Osteopathc Medicine, and her B.S. in Biophysiology at Eastern Michigan University, where she graduated Magna Cum Laude.
She had a successful private practice at North Bend Medical Center for nine years before establishing her current private practice, Rogue Valley ENT & Facial Plastics.
Licensure and Board Certification includes:
- Board Certified Otolaryngology and Facial Plastic Surgery (1997)
- National Board of Medical Examiners, Parts 1-3
- License held, State of Michigan 1992 – 1997, State of Oregon 1997 – present
Dr. Kozak’s personal interests include sports (especially tennis, racquetball and weight training), traveling, music, theater, camping, fishing and oil painting. She is married and she and her husband have a beautiful daughter.
